What do I need to bring to my appointment?

Bring in your most recent eye glasses and/or contact lenses. Please also bring the insurance card for the patient having the examination so we can bill your insurance for all eligible expenses. Date of birth and social security numbers of the patient (as well as the person who is responsible for the account the insurance is under) are also typically needed to verify coverage with insurance companies.
